All vendors, excluding those entered for credit card purchases, are required to have a W-9 on file with the district. A Conflict-of-Interest Questionnaire Form (CIQ) is also required to be on file if a known conflict of interest exists. Texas Local Government Code Chapter 176 (Disclosure of Certain Relationships with Local Government Officers) applies to contracts of the district. The General Manager may impose reasonable insurance requirements on any contractor, vendor or consultant, depending upon the nature of the contract and the work to be performed.
